Job summary

United Community Action Network (UCAN) in Southern Oregon seeks a Veteran Case Manager to help homeless and unstably housed veterans access housing and achieve stability across Douglas County. You will provide outreach, interview clients, assess needs, and connect them with housing resources.

This role requires a 2-year degree or equivalent experience, driving, and strong data-entry skills. UCAN offers a generous benefits package and supportive environment to empower clients and staff alike.

Qualifications

• Associates degree OR four years experience in human services.
• Equivalent combination of education and experience considered.
• Knowledge of community resources and poverty-related issues recommended.
• Ability to communicate effectively and maintain confidentiality.

Responsibilities

• Provide outreach services to homeless population including veterans and families.
• Interview clients to assess needs and refer to housing resources.
• Assist with obtaining housing eligibility and necessary documents.
• Enter data into HMIS and generate program reports.
• Maintain records and reports and coordinate with partners and landlords.
