Qualifications

• Bachelor's degree in human services required or related field.
• Minimum 2 years of experience in case management, preferably in homelessness.
• Strong interpersonal and communication skills.
• Ability to work with diverse populations and partners.

Responsibilities

• Assess and intake clients to determine program eligibility and needs.
• Navigate healthcare and coordinate care for veterans.
• Provide ongoing case management and housing stability planning.
• Assist with housing placement and access to subsidies.
• Administer financial assistance for rent, utilities, and related expenses.
• Perform crisis intervention and connect to mental health resources.
• Advocate for veterans' benefits and community supports.
• Document program data and generate regular outcome reports.
