Job summary

Mid-Willamette Valley Community Action Agency seeks a Veteran Housing Coordinator to interact with landlords and clients to negotiate rental opportunities and resolve housing barriers for veterans and their families who are homeless or at-risk. The role aims to develop individualized action plans for housing stability and self-sufficiency.

Qualifications

• Bachelor's Degree in Social Services or a related field.
• Two years case management or social service experience.
• Equivalent combination of education and experience may be considered.
• Experience working with veterans preferred.

Responsibilities

• Assess participant eligibility and gather documentation as required.
• Collaborates with Veteran Services Officer team to provide supports and advocacy for positive tenancy.
• Coordinates with Salem Housing Authority and partnering programs to provide financial assistance and supports.
• Completes participant assessments for continued eligibility and needs.
• Develops and coordinates individualized action plans.
• Collaborates with Peer Support Specialist to coordinate services.
• Monitors participant progress and maintains detailed case notes.
• Maintains organized participant files.
• Assists participants in locating and stabilizing housing.
• Develops and maintains relationships with landlords.
• Identifies needs for Barrier Removal Funds (security deposits, utilities, docs, etc.).
• Works with Salem Housing Authority to coordinate financial assistance.
• Networks with community agencies for services and resources.
• Follows up with agencies that committed assistance.
• Provides information, referrals, and advocacy as needed.
• Inputs client data in HMIS and MOTS.
• Prepares data and evaluation reports as required.
• Assists Housing Assistance Program Coordinator with data management and reporting.
• Participates in Community Outreach with local organizations and groups.
