Volunteers of America Colorado is seeking a Case Manager for the Back Home SSVF program to stabilize housing for veterans. You will conduct needs-based assessments, engage in outreach, provide case management, and coordinate services with program partners.

Qualifications

• Bachelor’s degree in human services, social work, or related field or equivalent experience.
• Minimum one year direct experience in human services.
• Colorado driver’s license and automobile insurance required; personal vehicle for travel.

Responsibilities

• Provides case management services to VOA program clients.
• Conducts needs-based assessments and engages in outreach.
• Coordinates wrap-around services and connects veterans to benefits and referrals.
