Community Partnership of Southeast Missouri (CPSEMO) is seeking a Health Care Navigator to support Veterans and their families in the SSVF program. The role helps enroll and connect Veterans with medical, behavioral health, and substance use treatments through VA and community providers.

This position focuses on rural areas with transportation challenges, coordinating with Case Managers to integrate health needs into housing stabilization plans and ensuring timely access to care.

Qualifikationen

• High school diploma or equivalent.
• Experience helping individuals or families access health care, housing, or social services.
• Strong organizational and communication skills.
• Ability to work with diverse populations and maintain confidentiality.
• Ability to work independently and with a multidisciplinary team.
• Reliable transportation to travel across CPSEMO's 14-county area.

Aufgaben

• Assess health-related barriers affecting housing stability.
• Identify gaps in medical, behavioral health, and substance use treatments.
• Assist Veterans in navigating health care systems and accessing services.
• Coordinate with Veterans, Case Managers, VA personnel, providers and partners.
• Help Veterans understand health care resources and eligibility requirements.
• Coordinate referrals to medical, behavioral health, dental, and other services.
