Job summary

Community Partnership of Southeast Missouri (CPSEMO) seeks a compassionate SSVF Case Manager to support Veterans and families facing homelessness across CPSEMO's 14-county rural service area. You will conduct intake, determine eligibility, develop housing stabilization plans, coordinate services, and engage landlords to secure safe, sustainable housing, using Housing First and Rapid Resolution approaches.

Qualifications

• High school diploma or equivalent.
• Experience working with individuals or families experiencing homelessness, housing instability, poverty, or other significant barriers to self-sufficiency.
• Strong communication, organization, and problem-solving skills.
• Ability to establish trusting and professional relationships with Veterans and their families.
• Ability to maintain confidentiality and exercise sound professional judgment.
• Ability to work independently while also functioning as part of a team.
• Ability to maintain accurate and timely documentation.
• Reliable transportation and ability to travel throughout CPSEMO's 14-county service area.
• Valid driver’s license and acceptable driving record.

Responsibilities

• Provide intake and eligibility determination for SSVF services.
• Develop individualized Housing Stabilization Plans with each enrolled household.
• Conduct housing searches and engage landlords and property management.
• Coordinate Temporary Financial Assistance (TFA) and document outcomes.
• Assist participants in accessing VA benefits, healthcare, employment, and other community resources.
• Maintain HMIS documentation and coordinate with Coordinated Entry as required.
• Provide short-term follow-up and support to maintain housing stability.
• Collaborate with Legal services, case workers, and community partners as needed.
