Qualifications

• Bachelor’s degree or equivalent combination of education and experience.
• Effective written and verbal communication skills.

Responsibilities

• Conducts non-clinical assessments with Veterans and interdisciplinary team to identify and address potential barriers to care; creates a Health Care and Housing Stability Plan.
• Identifies appropriate referrals for Veterans including housing financial benefits and transportation.
• Monitors and documents client contacts, needs, goals, progress, and significant occurrences per grant guidelines and PHC policies.
• Works with SSVF team and community providers to ensure Veteran centered care and improve care coordination.
• Identifies health education needs and provides materials meeting household literacy levels.
• Communicates with other agencies respecting client information and releases of information as needed.
• Builds rapport with Veterans and respects preferences in communicating with healthcare team.
• Participates in developing the Veteran’s Housing Stability Plan with the team.
• Collaborates with community providers to maximize VA and community resources use.
• Ensures clients have transportation for appointments per PHC policy; assists as needed.
• Performs outreach at community venues and serves as liaison between PHC and community resources.
• Performs other duties as assigned.
