Qualifikationen

• Bachelor’s degree in Social Work, Human Services, Psychology, Gerontology, Nursing, Rehabilitation Counseling, or related field required.
• Experience in case management, care coordination, or aging/veterans services preferred.
• Licensure/certification preferred but not required; valid driver’s license and transportation required.

Aufgaben

• Provide person-centered counseling and care coordination for Veterans in the VDC program.
• Develop and monitor individualized spending plans aligned with VA guidelines.
• Coordinate services among Veterans, caregivers, direct workers, and VAMC staff.
• Maintain accurate documentation in electronic health records and program systems.
• Advocate for Veteran choice, independence, and community inclusion.
• Collaborate with supervisors, partners, and interdisciplinary teams.
