Job summary

Transitions Mental Health Association is seeking a Case Manager - Peer Support (Veteran's Only) for the VetWell program in San Luis Obispo. This full-time role provides trauma-informed case management and peer mentorship to veterans with complex mental health needs.

You will work with clinicians and support clients toward independent living, linking them to community resources and services. A veteran status requirement and comprehensive benefits are included.

Qualifications

• Must have at least a bachelor’s degree or equivalent.
• Experience with trauma-informed care is preferred.
• Veteran status required by program grant guidelines.

Responsibilities

• Provide case management and peer support for an assigned group of clients.
• Assist clinicians in comprehensive assessments and treatment planning.
• Support crisis intervention and emotional coping strategies for clients.
• Coordinate community resources and services for independence goals.
• Facilitate client education, life skills, and relapse prevention.
• Document services and maintain client records and grant reporting.
