Transitions Mental Health Association is seeking a Case Manager- Peer Support for the San Luis Obispo Vet Well program. You will work with Veterans and their families to coordinate care, provide symptom management and community connections.

You will collaborate with clinicians to assess mental health history, educate families, and promote trauma-informed care while supporting independent living and vocational goals within a supportive team environment.

Qualifications

• BA/BS in a related field or CADAC/CPRP certification or equivalent experience.
• Strong interpersonal and communication skills are essential.
• Ability to manage time and prioritize tasks in a team setting.

Responsabilités

• Provide case management and peer support for an assigned group of clients with coordinated treatment goals.
• Assist clinicians in assessments and ongoing symptom evaluation.
• Support clients in coping skills, independence, and community integration.
• Facilitate connections to community resources and support groups.
• Help clients set and pursue vocational goals and job coaching when appropriate.
• Complete program paperwork and document client progress timely.
