Job summary

The Salvation Army Southern California is seeking a full-time Veterans Services Case Manager at the Owens Campus to support homeless and at-risk veterans and their families with intake, assessments, housing referrals, and follow-up services.

The role requires a Bachelor’s degree in Human Services, strong communication, and the ability to work with diverse partners; experience with HMIS/SQUARES is a plus. You will conduct outreach across local communities and coordinate care with VA personnel.

Qualifications

• Bachelor’s degree in a Human Services discipline; 4 years related experience may substitute.
• Experience with homeless and/or at-risk veterans and families preferred.
• Strong oral and written communication skills.
• Proficiency in Microsoft Office Software (Word, Excel, etc.)
• Valid driver’s license with clean driving record.

Responsibilities

• Manage up to 15 cases.
• Conduct intake, assessments, and initial service planning.
• Provide referrals and support for housing placement.
• Enter data in HMIS/SQUARES and ensure documentation compliance.
• Conduct outreach in Summerlin, Centennial Hills, and Henderson.
• Provide direct case management services to homeless and at-risk veterans and their families.
• Screen veterans’ eligibility for the program and facilitate the intake process.
• Coordinate care with VA personnel and others on behalf of participants for eligibility documentation, mental and physical health services.
• Attend all scheduled team meetings and trainings.
• Other duties and special projects as assigned.
