The role works closely with Veterans and families, ensuring timely, Veteran-centered care and collaboration with VA staff and community providers across settings as needed.

Qualifications

• Strong communication with Veterans, families, and teams.
• Maintain HIPAA confidentiality and professional boundaries.
• Proven ability to coordinate care across disciplines.

Responsibilities

• Actively communicate with Veterans, families, and team members.
• Coordinate care with VA staff and community providers.
• Complete documentation and care plans for each Veteran.
• Develop and monitor comprehensive care plans and outcomes.
• Meet deadlines for contract reporting and data submission.
