Volunteers of America Northern California & Northern Nevada, Inc. is seeking a Health Care Navigator for the Veterans Services Team in Mather, CA.

You will connect veterans with VA and community health resources, provide case management, education, and coordinate care across providers to support housing stability. The role requires a Master’s in social work or psychology (licensure preferred) and experience in social services, with strong communication and teamwork skills.

Qualifications

• Master’s degree in social work or psychology preferred; licensure preferred.
• One year experience in a social service environment required.
• Experience in outreach and making referrals to services preferred.
• Proficient with PC and Apple devices, web searches, MS Word, email, and remote platforms like Zoom or GoToMeeting.
• Broad knowledge of local services; veteran-focused experience is a plus.

Responsibilities

• Conduct assessments in collaboration with the interdisciplinary team.
• Identify barriers to health care and assist in planning care.
• Assist clients in communicating health care preferences for shared decision making.
• Develop tailored health care plans and monitor progress.
• Maintain thorough documentation and coordinate referrals to VA and community programs.
