Job summary

VOA Colorado is seeking a Health Care Navigator to connect Veterans with VA and community healthcare benefits, providing case management and care coordination. This role collaborates with interdisciplinary teams and engages with Veterans experiencing homelessness to facilitate access to services and adherence to care plans.

As part of the SSVF program, you will reach out to Veteran households across the service area, coordinate referrals, and advocate for Veteran-centered care while ensuring

Responsibilities

• Coordinate referrals to VA and community health programs.
• Provide non-clinical case management and health education.
• Assist Veterans with care plan adherence and scheduling.
• Maintain thorough, compliant documentation and communication with care teams.
• Advocate for Veterans and coordinate housing, benefits, and transport as needed.
