Job summary

Northeast Indiana Veterinary Emergency & Specialty Hospital (NIVES) in Fort Wayne, IN is seeking a Discharge Coordinator to support the discharge process for our emergency and specialty hospital. You will prepare medications, organize discharge materials, and coordinate with medical and client service teams to ensure accurate and timely patient discharge.

Qualifications

  • High school diploma or equivalent.
  • Experience handling medications or coordinating detailed workflows in a veterinary, pharmacy, or healthcare setting.
  • Strong attention to detail and the ability to perform medication calculations accurately.
  • Organization and the ability to adjust priorities in a busy hospital.
  • Clear, compassionate communication with clients and coworkers.
  • Good judgment about when to bring a question or concern to a veterinarian.

Responsibilities

  • Count, measure, package, and label medications according to prescriptions and hospital procedures.
  • Process prescription requests and refills, maintain records, and coordinate with outside pharmacies.
  • Monitor upcoming discharges and organize medications, written instructions, and follow-up information.
  • Collaborate with veterinarians, technicians, and client service teams to resolve discharge needs.
  • Communicate discharge readiness to clinical and client service teams.
  • Review medication labels and instructions with clients per hospital policy.
  • Report discrepancies or safety concerns and follow medication verification procedures.
  • Refer medical questions or concerns to a veterinarian when needed.
