• Coordinate patient updates, treatment plans, admissions, discharges, and transfers with the medical team.
• Anticipate client needs, address concerns, and resolve complaints professionally.
• Support clients through stressful situations and difficult conversations with empathy.
• Navigate interactions and de-escalate when needed while following policies.
• Ensure accurate information exchange between clients and the medical team.
• Identify recurring issues and suggest process improvements to boost satisfaction.
