Qualifications

• Must hold an Advanced Practitioner license in the state of practice.
• 2 years nephrology clinical experience is a strong plus.
• Excellent written and verbal communication skills.

Responsibilities

• Perform physical examinations and diagnose health issues.
• Order and interpret diagnostic tests; develop and initiate treatment plans.
• Prescribe medications and manage medications within scope.
• Educate patients and families on health problems, care plans and home care.
• Coordinate with nephrologists, care teams, and hospital discharge teams.
• Maintain electronic medical records and ensure continuity of care.
