U.S. Renal Care in Syracuse, NY is seeking an Advanced Practitioner to lead Chronic Disease Management and End-Stage Kidney Disease care within our clinical pathway framework. You will perform comprehensive visits, patient education, and coordinated care with nephrologists and hospital teams.

This role requires current Advanced Practitioner licensure, nephrology exposure, and strong communication skills. Full-time position with competitive benefits and a collaborative, inclusive culture.

Qualifications

Responsibilities

• Complete CDMVs and ESKD visits within clinical pathways.

• Collaborate with nephrologists, dialysis teams, hospital staff and PCPs.
• Ensure accurate documentation and EMR updates.
• Promote guest customer service standards and teamwork.
• Maintain regular and reliable attendance.

SUMMARY

Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.

Primary duties include

• Completion of Chronic Disease Management Visits (CDMVs), whether standalone or integrated within another VBC clinical pathway
• ESKD rounding, including limited and comprehensive visits, under the guidance of practice nephrologists
• ESKD holistic patient care management including but not limited to patient activation, depression, high risk of hospitalization, transition of care and co-morbid medical issue oversight
• ESKD patient education
• ESKD comprehensive disease management annual visits
• Care collaboration with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination team, PCPs and sub-specialists as clinically indicated
• CKD clinical care and kidney disease education
• Other duties and tasks as assigned

Functions performed during above duties

• Perform physical examinations of patients to evaluate health and diagnose illnesses
• Order and assess diagnostic tests, such as x-rays and laboratory tests
• Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary
• Prescribe medications
• Document medical information of patients and maintain electronic medical records
• Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors
• Request consultation or referral with other health care providers, such as physicians and clinics, collaborate with Kidneylink RN Care Manager
• Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.
• Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.
• Regular and reliable attendance is required for the job.

Qualifications

Qualifications/Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Requirements include

• Current license to practice as an Advanced Practitioner in the state performing duties
• Ideal candidate will have a firm understanding of the protocols, policies and operations of a physician practice.
• Must have necessary specialty certification 2 years clinical work experience Nephrology experience a strong plus.
• Excellent written and verbal communication skills

Pay Range
