Job summary

Diversicare is a post-acute care provider seeking compassionate CNAs to support residents with safe transportation and accompany them to medical visits. Our team values trust, respect, compassion and strong communication to deliver Service Excellence across our centers.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit: sign in resident, present documents.
• Before leaving doctor’s office, obtain resident’s documents and new prescriptions and verify doctor’s orders.
• Return to the facility and report to RCM with any questions about the visit.
• Perform other duties as required and assigned.

If follow-up appointment necessary, obtain appointment cards.5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.6. Performs other duties as required and assigned.
