Job summary

Diversicare Healthcare Services, LLC is seeking a dedicated Certified Nurse Assistant (C.N.A.) to support residents with daily care and safe transportation to medical visits. You will accompany residents, ensure documentation is in order, and assist with prescriptions and orders while maintaining a compassionate, professional standard.

Qualifications

• Certified Nurse Assistant (C.N.A.) required.
• Valid state driver’s license.
• Clean driving record and clean criminal record.
• Any other state required certifications.

Responsibilities

• Assure resident safety when transporting to/from doctor or hospital.
• Accompany residents on medical visits; stay with them until complete.
• Observe visit guidelines and present documents to staff.
• Obtain new prescriptions and doctor’s orders as needed.
• Ensure return to facility and report to RCM with visit details.
• Perform other duties as required and assigned.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit:
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to make sure doctor has written on the doctor’s order form.
• If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications
