Job summary

Diversicare is seeking a dedicated Certified Nursing Assistant to support post-acute care residents in our Alabama facilities. You will safely transport residents to appointments, accompany them during visits, and ensure all documents are properly handled at each step.

Qualified CNAs with a valid license and clean driving record are encouraged to apply. Excellent benefits and growth opportunities are offered within a team-focused environment.

Qualifications

• Certified Nursing Assistant (C.N.A.) with valid certification.
• Must have valid state driver's license.
• Able to assist ambulatory and non-ambulatory residents with transfers.

Responsibilities

• Ensure resident safety during transport to medical visits.
• Accompany resident on medical visits and stay with them until visit is complete.
• Sign in resident on arrival and present documents to office staff.

Responsibilities

• 1. Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• 2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• 3. Observe the following guidelines at each visit: a. Sign in resident upon entrance. b. Present resident’s documents to office personnel.
• 4. Before leaving doctor’s office, please observe the following: a. Obtain resident’s documents. b. Obtain any new prescriptions. c. Check to make sure doctor has written on the doctor’s order form. d. If follow-up appointment necessary, obtain appointment cards.
• 5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• 6. Performs other duties as required and assigned.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications
