Job summary

Diversicare is seeking a compassionate Certified Nurse Assistant to transport and accompany residents to medical appointments and to assist with daily care at our facility.

You will ensure safety during transfers, accompany residents to visits, manage documents and prescriptions, and report back to the nursing team. A valid CNA certification and clean driving/criminal records are required.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit: sign in resident upon entrance; present resident’s documents to office personnel.
• Before leaving doctor’s office, obtain resident’s documents and any new prescriptions; check for doctor’s order.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.
