Job summary

Diversicare Healthcare Services, LLC is hiring a dedicated CNA to assist residents with safe transfers and transportation to medical appointments. You will accompany residents, maintain safety, and ensure documentation is properly handled.

The role requires a valid CNA certification, a clean driving record, and a commitment to compassionate, patient-focused care within a facility setting.

Qualifications

• Must be able to help ambulatory and non-ambulatory residents transfer safely.
• Valid state driver's license required.
• Certified Nurse Assistant (C.N.A.) certification required.
• Clean driving record and clean criminal record.

Responsibilities

• Assure resident safety during transport to medical visits and back.
• accompany residents on medical visits and stay with them until completion.
• Record and present resident documents to office staff.
• Obtain prescriptions and appointment cards as needed.
• Report visit details to the Residence Care Manager on return.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit:
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to make sure doctor has written on the doctor’s order form.
• If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications
