Job summary

Diversicare Healthcare Services, LLC is looking for compassionate caregivers to assure the safety of residents during medical visits and assist them as needed. This role requires physical ability and qualifications as a Certified Nurse Assistant.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer.
• Possess a valid state driver's license and clean driving record.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Have a clean criminal record and any other required state certifications.

Responsibilities

• Assure the safety of each resident during transport to medical visits.
• Accompany residents on all medical visits until completion.
• Sign in residents upon entrance and manage their documents.
• Report to RCM after visits and answer questions regarding them.
• Perform other assigned duties as required.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit:
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to make sure doctor has written on the doctor’s order form.
• If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications
