Job summary

Conviva Senior Primary Care, a Humana subsidiary, seeks a Primary Care Physician to join our on-site Callahan/Jacksonville clinic in Florida. You will report to the Associate Medical Director and serve an adult patient population with a focus on value-based care and preventive health.

The role features a 40-hour work week, 12–15 daily patient load, and longer 45-minute visits when needed, with on-site duties in a supportive team environment.

Qualifications

• MD or DO required and eligible to practice in Florida.
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine.
• Valid, unrestricted license in Florida (or willingness to obtain).
• Current, unrestricted DEA license.
• TB screening conducted.
• Minimum 2+ years in value-based care.
• 1+ year managing Medicare Advantage panels.
• Language proficiency ILR assessment if non-English speaker.

Responsibilities

• On-site at the Callahan, FL clinic and report to the Associate Medical Director.
• See 12-15 patients per day with 45-minute appointments as needed.
• 40-hour work week, Monday–Friday, 8:00am–5:00pm.
• Provide value-based care and coordinate with Humana/CenteraWell partnerships.
