Job summary

Conviva Senior Primary Care, a Humana subsidiary, is seeking a primary care physician to join our on-site Coconut Creek, FL clinic. You will report to the associate medical director and work Monday–Friday 8:00am–5:00pm, delivering comprehensive outpatient care to adults and seniors.

We emphasize team-based care, longer patient interactions, and work-life balance with PTO, CME time, and a bonus incentive plan. Florida license and board certification qualifications apply.

Qualifications

• Two years' or more experience in primary care, geriatric care, or managing chronic conditions

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Work with interdisciplinary teams to ensure holistic care.

Skills

Bilingual: Portuguese & English

Education

Doctor of Medicine or Osteopathic Medicine

Board certification or board eligible in family medicine, internal medicine, or geriatric medicine

Florida license (or willing to obtain)

DEA license (or must obtain)
