Job summary

Humana Inc. through CenterWell / Conviva Senior Primary Care seeks a Primary Care Physician for a brand-new Port Salerno, FL clinic. You will deliver comprehensive in-person care, focusing on social determinants of health and senior wellness in a dedicated team.

The role reports to the Associate Medical Director and offers 40 hours weekly with 45-minute patient visits and an emphasis on direct patient care in a clinic setting.

Qualifications

• Medical degree required (MD or DO) and board certification/eligibility.
• Active, valid license in the state of employment (or willingness to obtain).
• Current, unrestricted DEA license.
• TB screening required if selected.

Responsibilities

• Provide direct patient care in the clinic with an emphasis on senior-focused primary care.
• See an established patient panel with longer appointments (45 minutes).
• Work onsite at Conviva Delray Beach Center in Delray Beach, FL and Port Salerno area as assigned.
