Job summary

Conviva Senior Primary Care, part of Humana, is launching a brand-new clinic in Port Salerno, Florida. As a Primary Care Physician you will provide hands-on, patient-centered care for seniors, in an onsite setting at the Conviva Delray Beach Center in Delray Beach.

The role emphasizes longer 45-minute visits, a manageable daily panel of 14–17 patients, and a focus on social determinants of health. Reporting to Associate Medical Director, 40 hours a week, with competitive benefits.

Qualifications

• MD or DO degree required.
• Board certified or board eligible in geriatric, family or internal medicine.
• Valid, unrestricted license in the state of employment (or willing to obtain).
• Current, unrestricted DEA license.
• TB screening is required if selected for the role.

Responsibilities

• Provide hands-on, patient-centered care addressing needs in the exam room and beyond social determinants of health.
• See an onsite, senior-focused patient panel with 14–17 patients per day and longer 45‑minute visits.
• Spend 100% of time on direct patient care, with time for administrative tasks related to care.
• Report to Associate Medical Director; work onsite at Conviva Delray Beach Center in Delray Beach, FL.
• Join an experienced team in a brand-new Port Salerno clinic setup.
