Job summary

Conviva Senior Primary Care, a subsidiary of Humana Inc., seeks a full-time Primary Care Physician for our Orange Park center in Jacksonville, FL. You will deliver direct patient care in a value-based model, while collaborating with an experienced team and maintaining thorough documentation.

Qualifications

• Doctor of Medicine (MD) or Osteopathic (DO) degree.
• Board certified or board eligible in Family Medicine or Internal Medicine.
• Geriatrics specialization is a plus.
• Florida license or willingness to obtain license.
• Current, unrestricted DEA license.

Responsibilities

• Provide clinical care to center patients.
• Coordinate referrals and care with the care team.
• Document medical records in a timely manner and support coding quality.
