Job summary

gobravvo is seeking a dedicated Primary Care Physician (MD/DO) to join a value-based care network in Peoria, IL. You will provide comprehensive primary care, collaborate with a multidisciplinary team, and emphasize preventive medicine and chronic disease management.

Qualifications

• MD or DO degree with board certification or eligibility in Internal Medicine or Family Medicine.
• Active, unrestricted medical license in the United States.
• Fellowship in Geriatrics preferred but not required.

Responsibilities

• Provide comprehensive primary care to a diverse patient panel.
• Deliver value-based care with emphasis on preventive medicine and chronic disease management.
• Collaborate with a multidisciplinary team including scribes, assistants, and social workers.
• Utilize EMR systems (Canopy, Greenway) for documentation and care coordination.
• Participate in quality improvement initiatives and patient engagement activities.
