Job summary

gobravvo is seeking a dedicated Primary Care Physician (MD/DO) to join its value-based outpatient clinic. You'll provide compassionate, preventive, and chronic care for seniors in a team-based setting, with support from care coordinators and medical assistants.

Qualifications

• Medical degree (MD or DO) from an accredited institution.
• Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) required.
• Fellowship training in Geriatrics is a plus, but not mandatory.
• Current, active, and unrestricted medical license to practice.
• Valid DEA registration.
• Proficiency in office-based procedures including suturing, joint injections, skin biopsies, and basic wound care.
• Commitment to compassionate, patient-centered care for a diverse senior population.
• Excellent communication and teamwork skills.
• Strong computer literacy and ability to utilize electronic health records efficiently.

Responsibilities

• Conduct comprehensive assessments and manage acute and chronic conditions.
• Develop, implement, and monitor individualized care plans for each patient.
• Manage a patient panel size with a maximum of 650 patients, typically seeing 10-12 patients per day.
• Collaborate with a multidisciplinary care team including medical assistants, schedulers, referral specialists, and leadership support.
• Utilize office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
• Participate in call rotation as required by clinic staffing levels.
• Engage in ongoing quality improvement initiatives including HEDIS and patient satisfaction improvements.
• Promote patient education, preventive health, and community wellness programs.
• Maintain accurate and timely documentation in the electronic medical record system.
