Job summary

HarmonyCares is seeking a Physician Leader Primary Care to oversee clinical pod performance and lead a team of APPs and physicians. This physician-driven, physician-led model emphasizes quality, patient safety, and collaboration across the pod and market team.

The role requires a medical license, residency, and DEA; leadership experience with chronic disease management is preferred. Competitive benefits and a focus on home-based care are offered.

Qualifications

• Doctor of Medicine or Osteopathy required.
• Active CPR Certification.
• Unrestricted DEA.
• Unrestricted Medical License and board certification for hires after Jan 1, 2026.
• Residency completion required.
• Bachelor’s Degree.

Responsibilities

• Act as on-site supervising physician for APPs and POD clinicians.
• Manage collaborative practice agreements within state rules.
• Lead APP clinical development and evidence-based care.
• Participate in high-risk patient reviews and care planning.
• Drive quality metrics and care improvements for the pod.
