Job summary

HarmonyCares Primary Care Medical Group in Grand Rapids and Kalamazoo seeks a Value Based Care Physician Leader to guide a growing provider team in delivering high-risk Medicare patient outcomes through HEDIS-focused care, in-home visits, and collaborative leadership with physician and regional leaders.

As Medical Pod Leader, you will oversee clinical performance, ensure accurate diagnosis and documentation, maintain team dynamics, and serve as collaborating physician for APPs, with a dyad

Qualifications

• Must be a Doctor of Medicine or Osteopathy.
• Unrestricted DEA.
• Completion of an accredited Residency Program.
• Board certification is required for all newly hired or promoted on or after January 1, 2026; must obtain and maintain certification in accordance with organizational and applicable regulatory standards.
• 3 years of experience as a Physician collaborating with Advance Practice Providers.
• Experience in Geriatrics or complex patients.

Responsibilities

• The Medical Pod Leader will be the clinical leader responsible for the clinical performance of clinical pod team members – to include: Accurate diagnosis and documentation, quality, patient experience, affordability, patient safety, clinical team dynamics, and provider productivity.
• The pod leader will be tasked with developing and maintaining close working relationships with all clinical team members in the clinical pod.
• The pod leader will also function as the collaborating physician for associated pod advanced practice providers (APPs).
• This position will have a dyad partnership with the Market Manager and market office staff.

Job Description

Required Knowledge, Skills and Experience

• Must be a Doctor of Medicine or Osteopathy
• Must maintain a valid driver’s license and maintain a good driving record
• Active CPR Certification
• Unrestricted DEA
