Job summary

HarmonyCares is seeking a Geriatric Physician Leader to oversee clinical pod teams delivering in-home primary care under a value-based care model. You will supervise APPs, ensure accurate diagnoses and documentation, and drive quality and patient safety across the pod.

This role partners with market leadership to optimize care delivery and EMR usage. The ideal candidate has an MD/DO, valid medical license, and experience with geriatrics or complex patient populations, with a track record of

Qualifications

• Must hold Doctor of Medicine (MD) or Doctor of Osteopathy (DO) with active license.
• Residency training completed in accredited program.
• Experience leading clinical teams in geriatrics or complex patient populations.
• Proven ability to supervise APPs and collaborate with physicians.

Responsibilities

• Provide clinical leadership for pod team performance and patient safety.
• Oversee accurate diagnosis, documentation, and quality metrics.
• Coordinate APP development and ongoing clinical education.
• Lead high-risk patient care planning and care coordination.
• Ensure EMR efficiency and regulatory compliance.
• Collaborate with Market Manager and RMD on care improvement initiatives.
