Job summary

HarmonyCares is seeking a Value-based Physician Leader to oversee a primary care pod focused on high-risk geriatric patients in Kalamazoo. This physician supervises APPs, coordinates chronic condition management, and collaborates with a market team to improve quality and safety.

The role emphasizes provider productivity, documentation accuracy, and team-based care in a no-weekends setting. Qualifications include MD/DO, unrestricted license, DEA, residency, and board certification (post-2026).

Qualifications

• Must be a Doctor of Medicine or Osteopathy.
• Active CPR Certification.
• Unrestricted DEA.
• Completion of an accredited Residency Program.
• Unrestricted Medical License.
• Board certification required for hires promoted after Jan 1, 2026.

Responsibilities

• Act as on-site supervising Physician for assigned APPs; manage collaborative practice agreements per state.
• Collaborate with clinical team members (e.g., CHWs, Care Managers, Nurse Navigators).
• Lead APP clinical development and evidence-based care.
• Lead high-risk huddles and ensure care plans include all team members.
• Review Quality metrics and drive improvements in pod performance.
• Oversee clinical reviews, EMR efficiency, and patient care for assigned panels.
