Oak Street Health in Atlanta is seeking a Primary Care Physician to deliver equity-centered, value-based care to local Medicare populations within our neighborhood clinics. You will work with a multi-disciplinary team to improve outcomes and patient experience, focusing on quality care over volume.

Qualifications

• MD or DO graduate with board eligible or certified in Internal Medicine or Family Medicine.
• Active, unrestricted State Medical License.
• Active DEA license.
• U.S. work authorization.
• Experience in value-based care settings or geriatrics preferred.

Responsibilities

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain EMR data via canopy and greenway
• Collaborate with leadership to meet health quality goals
• Work with Practice Managers to direct the center care team
• Other duties as assigned
