Oak Street Health is seeking a Nurse Practitioner to join a value-based primary care team serving Medicare adults in outpatient settings. You will collaborate with physicians, lead care coordination, and support population health initiatives to improve patient outcomes.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• National certification in one of the listed NP specialties.
• Active, non-probationary state NP license.
• Active DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.

Education

Master of Science in Nursing / Doctor of Nursing Practice / NP Certificate Program

National certification in Family Nurse Practitioner or Adult-Gerontology Primary Care NP or Adult Nurse Practitioner or Gerontological Nurse Practitioner

Active, non-probationary state NP license
