Job summary

Oak Street Health is recruiting a Nurse Practitioner to provide value-based primary care in outpatient clinics for adults on Medicare. You will work with a physician-led team of medical assistants, nurses, and care coordinators to keep patients healthy, informed, and connected to their care plans.

The role features independent patient visits, wellness exams, chronic disease management, care coordination with specialists, and home visits as needed.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• National certification in one of the listed specialties (FNP/AGPCNP/ANP/GNP).
• Active, non-probationary NP license.
• Active DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments, with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership with the care team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
