Job summary

Oak Street Health in Raleigh, NC is seeking a Nurse Practitioner (Advanced Practice Provider) to deliver effective, value-based primary care to adults on Medicare. Our collaborative care team supports you to keep patients happy, healthy, and out of the hospital.

You will see patients independently within outpatient clinics, coordinate with physicians and specialists, perform annual wellness visits, and participate in care planning and population health initiatives.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP
• Active, non-probationary NP license in the US
• Active DEA license
• US Work Authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with physicians, specialists, facilities and agencies.
• Population health leadership in coordination with the care team.
• Phone triage and outreach support for patients.
• Educate patients on conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
