Job summary

HarmonyCares is seeking a Full-time Physician Leader to guide clinical pod teams with direct APP supervision. The role balances hands-on patient care with administrative oversight, supporting growth and quality initiatives across home-based primary care services.

The leader will coordinate with market and regional directors, oversee clinical documentation, and lead education for APPs while ensuring adherence to regulatory standards and high-quality, cost-effective care.

Qualifications

• Bachelor’s degree required; must be a MD or DO.
• Unrestricted medical license and active CPR certification.
• Board certification required for hires after Jan 1, 2026; ongoing certification maintained.
• Completion of an accredited residency program; DEA registration.

Responsibilities

• Act as on-site supervising physician for APPs and pod team; ensure clinical quality and documentation.
• Lead clinical development and Evidence-based care initiatives with the pod.
• Participate in high-risk huddles and ensure care plans include all care team members.
• Drive quality metrics, patient safety, and program productivity with leadership partners.
• Conduct chart audits and support EMR efficiency and care improvements.
• Mentor, develop, and perform performance reviews for direct reports.
