Harbor Health in El Paso, TX is seeking a Board-Certified Family or Internal Medicine Physician to join a team-based care model at the Timber Wolf location. You will provide primary care and preventive services within a collaborative care team focused on improving health outcomes, patient experience, and reducing waste.

Qualifications

• Texas medical license or ability to obtain one
• Board certification in Family Medicine or Internal Medicine
• Current DEA and CPR certification
• Valid Texas Driver's License
• Credentialing and privileging requirements
• Strong clinical judgment and diagnostic skills
• Patient-centered care delivery
• Excellent communication and interpersonal skills
• Proficiency with EHR systems
• Ability to work with diverse backgrounds
• Confidentiality and professionalism

Responsibilities

• Deliver high-quality primary and preventive care in-person, via telemedicine, and by phone
• Manage a patient panel focused on population health and individual outcomes
• Collaborate with interdisciplinary team to design and implement care protocols
• Participate in quality improvement initiatives to improve outcomes and patient experience
• Maintain accurate clinical documentation and use data-driven insights for decisions
• Contribute to development of clinical workflows in a team-based model
• Engage in professional development and evidence-based practice discussions
• Document and code all member contacts in the EMR
• Actively participate in care team meetings
• Support organizational initiatives and committees with a patient-centered approach
