Qualifications

• Texas medical license required.
• Board certification in Family Medicine or Internal Medicine.
• DEA and CPR certification.
• Valid Texas Driver's License.
• Ability to meet credentialing and privileging requirements.
• Experience with population health and data-driven care is a plus.

Responsibilities

• Deliver primary and preventive care via in-person, virtual, and phone visits.
• Manage a patient panel with focus on population health outcomes.
• Collaborate with care team to design member-centered care protocols.
• Engage in quality improvement to enhance outcomes and experience.
• Maintain accurate documentation in the electronic medical record.
