Harbor Health in Austin, TX is seeking a Board-Certified Family or Internal Medicine Physician to join our innovative, team-based care environment at our East Austin location. You will deliver primary care and preventive services as part of a care team redesigning healthcare to improve health outcomes and member experience.

Qualifications

• Board-certified in Family Medicine or Internal Medicine.
• Active Texas medical license or ability to obtain one.
• Current DEA and CPR certification.
• Valid Texas Driver's License.
• Ability to meet credentialing and privileging requirements.
• Excellent communication and interpersonal skills.
• Experience with population health and care coordination.

Responsibilities

• Deliver high-quality primary and preventive care through in-person, virtual, and telephone visits.
• Manage a panel of members with focus on population health and outcomes.
• Collaborate with interdisciplinary care teams to design member-centered care protocols.
• Participate in continuous quality improvement initiatives to enhance outcomes and experience.
• Maintain accurate clinical documentation and engage with data-driven insights.
• Contribute to workflows and care processes within a team-based model.
• Engage in professional development and team discussions.
