Qualifications

• Current Texas medical license or ability to obtain one.
• Board certification in Family Medicine or Internal Medicine.
• Current DEA and CPR certification.
• Valid Texas Driver's License.
• Ability to meet and maintain credentialing and privileging requirements.
• Strong clinical judgment and excellent diagnostic skills.
• Proven track record of delivering high-quality, patient-centered care.
• Excellent communication and interpersonal skills.
• Computer literacy with ability to navigate electronic health record systems.
• Ability to interact effectively with persons from diverse backgrounds.
• Sound judgment, flexibility to shift priorities, and commitment to confidentiality.

Responsibilities

• Deliver high-quality primary and preventive care through in-person, virtual, and telephone visits.
• Manage a panel of members with focus on population health and outcomes.
• Collaborate with interdisciplinary care team to design member-centered care protocols.
• Participate in continuous quality improvement to enhance outcomes and experience.
• Maintain accurate clinical documentation and use data-driven insights for decisions.
• Contribute to development and refinement of clinical workflows in a team-based model.
• Engage in professional development and evidence-based practice discussions.
• Document and code all member contacts accurately in the EMR.
• Function as an active member of the medical team and attend care team meetings.
• Contribute to organizational initiatives including clinical committees and education.
