Qualifikationen

• Current Texas medical license and board certification in Family or Internal Medicine.
• DEA and CPR certification.
• Ability to meet credentialing and privileging requirements.
• Strong clinical judgment and diagnostic skills.
• Track record of delivering high-quality, patient-centered care.

Aufgaben

• Deliver high-quality primary and preventive care through in-person, virtual, and phone visits.
• Manage a patient panel with focus on population health and outcomes.
• Collaborate with the care team to design and implement member-centered protocols.
• Participate in quality improvement initiatives to enhance outcomes and experience.
• Maintain accurate documentation and use data-driven insights for decisions.
• Contribute to development of clinical workflows in a team-based model.
• Engage in professional development and evidence-based discussions with the team.
• Document and code all member contacts in the EMR.
• Participate in care team meetings and organizational committees.
