Qualifications

• Current Texas medical license or ability to obtain one.
• Board certification in Family Medicine or Internal Medicine.
• Current DEA and CPR certification.
• Valid Texas Driver's License.
• Credentialing and privileging eligibility.
• Strong clinical judgment and diagnostic skills.
• Patient-centered, high-quality care delivery.
• Excellent communication and interpersonal skills.
• Proficiency with electronic health records.
• Ability to work with diverse backgrounds.

Responsibilities

• Deliver primary and preventive care in-person, virtual, and telephone visits.
• Manage a panel of members focusing on population health and outcomes.
• Collaborate with interdisciplinary care teams to design care protocols.
• Participate in quality improvement initiatives to enhance outcomes.
• Maintain accurate documentation and use data-driven insights.
• Contribute to workflow design within a team-based model.
• Engage in professional development and evidence-based discussions.
• Document and code member contacts in the EMR.
• Be an active member of the medical team and attend care meetings.
• Contribute to organizational initiatives and committees.
