Job summary

Astrana Health, Inc. is seeking a Practice Transformation Manager to advance value-based care across the San Jose/Santa Clara County market. This hybrid role supports ACO/IPA initiatives, engages providers, and drives performance improvements with regional teams.

The position reports to the VP of Strategy and Partnerships, requires strong data-driven decision skills, and coordinates with Finance, Coding, and Operations to optimize quality, utilization, and risk adjustment outcomes.

Qualifications

• Bachelor’s degree in Healthcare Administration, Business, Finance, or related field.
• At least 5 years of experience in provider network relations, contracting, provider data management healthcare operations, managed care, or related field
• Experience in provider relations, IPA, and ACO experience preferred

Responsibilities

• Serve as primary strategic liaison for ACO and IPA providers.
• Assess provider and market performance and identify improvement opportunities.
• Lead meetings with physicians and provider groups to present performance data.
• Develop engagement, onboarding, and adoption strategies for value-based care.
• Collaborate with care teams to support HCC documentation and risk adjustment.
• Manage complex cross-functional projects and initiatives.
• Provide data-driven analyses and presentations to leadership.
