Job summary

Astrana Health in NorCal seeks a Practice Transformation Manager (Value-Based Care) to drive performance across the ACO and IPA in the San Jose/Santa Clara County market. Reporting to the Vice President, Strategy and Partnerships, this role acts as a strategic advisor to providers and stakeholders.

You will lead value-based care initiatives, assess provider and market performance, present data, and partner with regional teams to improve quality, utilization, risk adjustment, and patient outcomes

Qualifications

• Bachelor’s degree in Healthcare Administration, Business, Finance, or related field required
• At least 5 years of experience in provider network relations, contracting, provider data management, healthcare operations, managed care, or related field
• Experience in provider relations, IPA, and ACO preferred
• Demonstrated experience analyzing data and making recommendations that impact business or operational outcomes

Responsibilities

• Serve as the primary strategic liaison for ACO and IPA providers
• Assess provider, practice, and market performance to identify improvement opportunities
• Conduct meetings with physicians, provider groups, practice administrators, and clinical staff to present performance data and influence improvements
• Develop and implement provider engagement and onboarding strategies for value-based care initiatives
• Lead provider change management and practice transformation
• Analyze practice operations and recommend improvements to wellness visit workflows, scheduling, quality programme execution, referral management, and other value-based care initiatives
• Evaluate performance improvement initiatives and adjust strategies based on outcomes
