Job summary

Astrana Health, Inc. is seeking a Practice Transformation Coach to drive clinical quality improvement and operational performance across provider practices in the Maryland and Northern Virginia region.

You will analyze practice performance, identify gaps, and develop targeted transformation strategies to improve quality outcomes and financial performance. Working with physicians and care teams, you will guide workflow redesign and support adoption of population health and performance improvement

Qualifications

• Bachelor's degree in nursing, healthcare administration, business, public health, or related field.
• Equivalently qualified by education and experience may be considered.
• 3+ years of analysis or provider/medical operations experience.
• Experience in value-based care, practice transformation, or quality improvement initiatives preferred.
• Demonstrated experience analyzing performance data and developing data-driven strategies.
• Experience working with provider groups, physicians, or clinical teams preferred.
• Licensure or certifications such as LVN/RN/CPC preferred.
• CPHQ, MHA, MPH, PMP, or Lean/Six Sigma certification preferred.

Responsibilities

• Serve as a strategic advisor to provider practices and assess clinical, operational, and financial performance.
• Prioritize engagements based on data and potential business impact.
• Develop and lead transformation plans to improve quality, workflow, and financial outcomes.
• Lead workflow redesign initiatives including EHR optimization and billing practices.
• Diagnose root causes of underperformance and develop data-driven solutions.
• Guide providers and care teams to adopt optimized workflows and sustain improvements.
• Collaborate with Market Operations, Quality, and Population Health to align on initiatives.
• Influence decision-making through data-driven insights and education.
• Act as a liaison between provider practices and leadership to share insights.
