Honest Health is looking for a Physician Practice Partner to build and sustain relationships with partner practices, guiding them through value-based care improvements. The role requires frequent travel and strong collaboration with clinical teams to drive performance and outcomes.

You’ll work remotely from the Buffalo/Rochester area, engaging PCPs, specialists, and health systems. Experience in practice management and data-driven performance is essential for success in this role.

Qualifications

Bachelor’s degree in healthcare administration preferred; 4+ years in practice management and/or as a physician specialist in a health system or IPA may substitute for a bachelor’s degree.
5+ years of experience in a healthcare environment with engagement of PCPs, specialists, and health systems.
Strong knowledge of clinic operations, medical office management, and clinical supervision and staff management.
Must be comfortable with remote work and travel up to 70% of the time.

Responsabilités

Build and maintain strong relationships with clinical partners and stakeholders.
Use in-person, electronic, and telephonic outreach to engage practices.
Onboard practices to Honest services and drive program implementation.
Educate practices on value-based care opportunities and cost efficiencies.
Be accountable for KPIs in utilization, cost of care, documentation, and quality.
Utilize data to develop performance strategies for value-based programs.
Communicate Honest programs to partner teams and coordinate performance tactics.
Serve as the primary contact for operational issues including back office and CRM inputs.
